Armodafinil for Night-Shift Sleepiness: Four Trial Checks
Sleep & NighttimeArticleCheck the study population, sleep test, missing observations and prescribing risks.

Bring your work rota, usual sleep times and medicine list to an appointment about night-shift sleepiness. Include unintended dozing and any difficulty getting home safely. These four checks can help you discuss armodafinil without overlooking the assessment and follow-up it requires.
Check who entered
The 12-week trial randomized 254 adults to armodafinil 150 mg or placebo. Participants had at least moderately severe chronic shift work disorder, three months of symptoms, daytime insomnia and laboratory sleep latency of six minutes or less. Of those randomized, 245 took treatment; 216 entered efficacy analyses.1
Define improvement
Mean nighttime sleep latency increased 3.1 minutes with armodafinil versus 0.4 with placebo (P<.001). This measured time to fall asleep in scheduled laboratory naps.1
Investigators rated 89/112 versus 61/104 at least minimally improved (P=.001). That threshold included small improvements.1
Read the limits
Final analyses used week 12 or the last available postbaseline observation carried forward. They did not include every randomized participant. Cephalon funded the trial. Diary-reported commuting accidents or near misses did not differ significantly; a sleep-test improvement does not establish driving safety.1
In a 2026 conference abstract, the AASM reported a conditional recommendation for armodafinil or modafinil in adults with excessive sleepiness from shift work disorder, with moderate-certainty evidence. Conditional recommendations require individual clinical judgment and discussion of preferences. This is an abstract, not a substitute for an individual prescribing review.2
Plan the prescribing review
US Nuvigil labeling includes excessive sleepiness associated with adult shift work disorder. It says the medicine does not replace adequate sleep and wakefulness may remain abnormal. Do not resume driving or hazardous duties until your clinician confirms adequate wakefulness. Ask how persistent sleepiness will be reassessed.3
Review psychiatric history, heart problems, blood pressure, pregnancy or breastfeeding and every medicine with the prescriber. The US label advises an additional barrier method or alternative non-hormonal contraception during treatment and for one month after stopping, because hormonal contraception may be less effective.3
A new rash, mouth sores, blisters or peeling skin requires stopping Nuvigil and contacting the clinician immediately. Swelling of the face or throat or trouble breathing needs emergency help. Ask the pharmacist to explain the Medication Guide before starting.3
Did I Do That in My Sleep? includes “Make the Account Specific”: describe what happened, when it began, whose observation it was and how it affected your day. Bring that account to assessment. If you are sleepy at the wheel, arrange a safe alternative to driving; do not wait for a medicine review to change that immediate decision.
This article is educational and does not replace assessment by your own clinician. Do not start armodafinil, adjust its timing or change prescribed treatment on the basis of a research report.
