Cataplexy or a Sleep Attack: What to Describe at a Visit

Separate muscle weakness from falling asleep when explaining daytime episodes.

  3. New weakness needs its own assessment

If you have been calling every daytime episode “crashing,” bring a more literal description to the appointment. Did you fall asleep? Did your muscles give way while you stayed aware? Use the words you have, then explain what happened.

The distinction to describe

What happened? What to tell the clinician
Muscle weakness during an emotional momentCataplexy can follow laughter or other emotions. Explain whether you remained aware and whether you fell.12
Falling asleep during the dayNarcolepsy can involve daytime sleepiness, difficulty staying awake and falling asleep suddenly. Describe what you were doing when sleep interrupted it.2
Both kinds of episodeReport each separately. A clinician assesses the pattern and decides what testing is needed.2

Write down the date, what happened immediately beforehand, what you remember and what someone else saw. If a person witnessed an episode, ask them to describe it in their own words. Avoid choosing a diagnosis for them to agree with.

Include what happens between episodes

Narcolepsy symptoms can include disrupted nighttime sleep, concentration problems and sleep paralysis when falling asleep or waking. The specialist asks about symptoms, lifestyle and sleeping habits. A symptom diary can help that conversation.2

Mention what the episode interrupted: a conversation, a work task or a meal. And tell the team if you have started avoiding an activity because you are worried about another episode. Put that concern on the appointment list alongside the physical description.

Royal Papworth explains that suspected narcolepsy can be investigated with an overnight sleep study and a Multiple Sleep Latency Test. Treatment for daytime sleepiness and treatment for troublesome cataplexy can involve different medicines. Ask which symptom a proposed treatment addresses.1

New weakness needs its own assessment

Do not assume new weakness is cataplexy. Seek emergency care for sudden weakness or paralysis, weakness after a head, neck or back injury, or weakness with speech, breathing or swallowing problems. One-sided facial drooping or arm weakness also needs emergency care. Call 911 or your local emergency number.3

For recurring daytime sleep episodes, arrange a medical appointment. Discuss safety at home and work, including activities where unexpected sleep or a fall would be dangerous.2

Did I Do That in My Sleep? includes a 14-Night Lights-Out Record and Visit Brief. Use these to organize sleep observations and questions alongside your episode notes. Bring the description you wrote, including the words you started with. See the sleep and nighttime playbook.
