4 Checks on Colchicine and Mouthwash for Canker Sores
Breath & MouthArticleThe yearlong comparison did not establish an advantage from adding colchicine.

If recurrent mouth ulcers disrupt eating or speaking, bring a symptom record and full medicine list to your appointment. Discuss these four points before adding colchicine.
Check what the comparison could establish
The UK trial randomized 106 adults to betamethasone mouthwash, oral colchicine or both, excluding overlapping systemic disease. There was no placebo. Guy’s and St Thomas’ sponsored it and supplied medicines.1
Without placebo, improvement alone cannot isolate a treatment effect. Ask how the clinician will judge improvement in your own case: ulcer frequency, painful days, eating difficulties or another agreed measure.
Separate improvement from added benefit
Among 86 yearlong completers, mean Ulcer Severity Score fell from 34.9 to 17.5. Neither colchicine arm was significantly better than betamethasone alone. This does not establish equivalence.1
Rinse schedules differed: betamethasone alone continued between attacks; the combination’s rinse was used only during attacks.1
Ask whose results remained in the analysis
Yearlong completers numbered 29/35 rinse, 26/35 colchicine and 31/36 combination recipients. Clinician blinding lasted three months. Six adverse reactions, all in colchicine groups, caused withdrawal. Completer analysis leaves missing participants’ outcomes uncertain.1
Before agreeing to months of treatment, ask about the review date, what would justify continuing and which symptoms should prompt earlier contact. Do not change an existing prescription based on this summary.
Check the tablet and rinse instructions separately
Recurrent aphthous stomatitis is off-label for this US colchicine product. The label contraindicates colchicine with strong CYP3A4 or P-glycoprotein inhibitors in people with kidney or liver impairment. Clarithromycin and cyclosporine are important examples of interacting medicines; potentially fatal toxicity has occurred.2
The label also warns about blood disorders and muscle toxicity. Give the prescriber and pharmacist your complete medicine list, including statins, and mention kidney or liver disease. Ask for an interaction check when another medicine is prescribed, rather than assuming a short antibiotic course is harmless.2
UK specialist pharmacy guidance describes betamethasone soluble tablets used as mouthwash as off-label and warns about accidental swallowing. The prescribed mouthwash is rinsed and spat out. Ask for written instructions specific to that formulation: a tablet leaflet describing another use can cause confusion. Do not substitute a skin preparation or devise a rinse recipe.3
An ulcer lasting longer than 3 weeks, one different from your usual sores, or one that bleeds needs a dentist or GP assessment under national guidance. Repeatedly treating it as an ordinary canker sore can delay that assessment.4
Is It My Breath? covers persistent mouth symptoms in “The Red-Flag List” and suggests bringing a written medicine list and symptom history to appointments.
