If you are alert long after you meant to sleep and then struggle through the morning alarm, another lecture about bedtime may feel unhelpful. Bring the actual pattern to a sleep appointment. Ask the clinician to explain what they think is happening and which observations would help.

Three assessment questions

  1. Which pattern should I record? NHLBI describes asking when, how long and how well you sleep, and sometimes using a sleep diary.2 Include both days with morning obligations and days when you can wake without an alarm.

  2. Do I need further assessment? A clinician may use actigraphy or sleep studies and consider other conditions affecting sleep.2 Ask what a proposed test would clarify in your case.

  3. What timing does treatment require? Treatment may involve light exposure, schedule changes and melatonin. NHLBI describes morning light therapy for moving sleep and wake times earlier.3 Ask for the timing of any recommended approach in writing.

Check before buying a light box

NHLBI advises discussing light therapy first if you have an eye condition or take medicines that increase light sensitivity. Melatonin can have adverse effects and interactions, and supplement dose and purity can vary.3 Have the team review the specific plan.

Ask how progress will be reviewed and what to report if the routine is difficult to follow. Keep those details beside your sleep notes.

Did I Do That in My Sleep? covers concerns such as night waking and snoring. Use it alongside the questions you bring to your sleep team. See the book.
