Eluxadoline for IBS-D: How the Trials Counted Improvement

Two phase 3 trials counted pain and stool improvement together. Their response percentages need that definition attached.

Eluxadoline trial response rates do not describe complete symptom freedom. The studies counted pain and stool improvement together.1

What counted as a response

Two placebo-controlled trials assigned 2,427 adults with IBS-D to 75 mg or 100 mg eluxadoline, or placebo, twice daily for 26 or 52 weeks. Furiex Pharmaceuticals funded them. These figures come from the original abstract.1

The prescribing information supplies the threshold: at least 30% less worst abdominal pain and a Bristol stool score below 5, together on at least 50% of days over 12 weeks. A day with pain improvement but no bowel movement could also count. The label reports that 66% of participants were women.2

How many met that threshold

At 12 weeks, the original abstract reported these combined responses for twice-daily treatment:1

  • IBS-3001: 23.9% with 75 mg, 25.1% with 100 mg and 17.1% with placebo.
  • IBS-3002: 28.9% with 75 mg, 29.6% with 100 mg and 16.2% with placebo.

For 100 mg, subtraction gives 8.0 and 13.4 additional responders per 100 participants, respectively. Most participants taking either dose did not meet the combined threshold. Missing that endpoint does not establish that someone had no improvement in any symptom.1

At 26 weeks, the 75 mg comparison in IBS-3001 was not statistically significant; both doses in IBS-3002 and 100 mg in IBS-3001 were. The trials do not support treating every dose and time interval as equivalent.1

Check eligibility before weighing benefit

The US label says do not take eluxadoline without a gallbladder. Other contraindications include prior pancreatitis, certain biliary or pancreatic disorders, severe liver impairment, chronic or severe constipation, bowel obstruction, and alcohol abuse or more than three alcoholic drinks daily. A prescriber must review the full list and other medicines.2

Pancreatitis has occurred after one or two doses. Stop the medicine and seek emergency care for new or worsening abdominal pain, including upper-right pain that may spread to the back or shoulder, with or without nausea or vomiting. For severe constipation, stop and contact the prescriber immediately. These warnings also matter when a gallbladder is present.2

Bring two symptom measures to the review

Pain and stool form need separate entries if you want to describe what changed. Add urgency and any constipation, then agree with the prescriber on a review date and what would justify continuing. The study threshold can explain the evidence; it is not an instruction to continue through adverse symptoms.

NIDDK3 describes diagnosis through symptom history, medical and family history, examination and selected tests for other conditions. Bleeding, black tarry stools, anemia or weight loss need assessment rather than being attributed automatically to IBS. A treatment response cannot substitute for that assessment.3

Bloated by Dinner includes a Belly Clock with timed pain, urgency, Bristol stool types and medicines. Bring those entries to the appointment. Keep warning symptoms out of the wait-and-log routine: they need attention when they occur.

Educational information. Eluxadoline requires a prescription and review of contraindications and interactions. New or worsening abdominal pain while taking it can require emergency care.
