If your clinician has confirmed a gastric bezoar, an unfamiliar name can leave you unsure what the proposed treatment is meant to remove. Ask for an explanation of the finding and the plan before turning an online treatment description into something to try at home.

What exactly has been found?

A bezoar is a tightly packed collection of partially digested or undigested material, most often in the stomach. Different types contain different material, including plant matter, hair or medicines. Upper endoscopy is usually used to confirm the diagnosis. Symptoms alone do not identify a bezoar.1

Ask: “What type do you think this is, and how was it confirmed?” Keep the examination report with the answer. If the material is not yet established, ask what further assessment is needed.

Why are there several treatment options?

Treatment can involve chemical dissolution, endoscopic removal or surgery. Merck notes that comparative randomized trials have not established the best option and that some cases require combined treatment. The choice needs a clinical discussion rather than a home experiment.1

Ask which option is proposed for you, why it fits the finding and what its risks and alternatives are. If dissolution is discussed, get the team’s own written instructions. Do not copy a drink, enzyme or medicine regimen from a treatment list online.

When might endoscopic removal be considered?

Merck describes endoscopic removal when a bezoar fails to dissolve or causes moderate to severe symptoms due to its size. The clinician may break it into pieces for extraction or passage. An endoscopy that establishes the diagnosis may also allow treatment.1

Ask whether the planned appointment is for diagnosis, treatment or both. Request the service’s preparation and recovery instructions, including any transport arrangements. Have the team specify what to do with your usual medicines.

Does a confirmed bezoar mean surgery?

Surgery is an option when dissolution or endoscopic treatment cannot be performed or has failed, and in certain complicated cases. It is not the automatic next step for every confirmed gastric bezoar.1

Ask what would lead the team to change your current plan and how they will check whether treatment has worked. Before leaving, request a contact route and instructions for seeking help if your symptoms change while treatment is being arranged.
