What Does an HDSS Sweat Score Describe?

Understand the questionnaire’s focus on tolerability and daily activities before discussing the result.

If you are handed a sweat questionnaire at an appointment, you may want to explain more than a single answer allows. Complete the question as instructed, then describe the situations behind your choice.

Ask which area the clinician wants you to consider. If more than one area troubles you, ask how the team wants that recorded.

The response combines tolerability and disruption

The four options progress from sweating that is never noticeable and never disrupts activities, through tolerable sweating that sometimes interferes, to barely tolerable sweating that frequently interferes, and intolerable sweating that always interferes.1

Think about the experience you want the clinician to understand. If you struggle to choose between responses, explain why. You could say: “This is tolerable on some days, but it often interrupts this particular activity. Which answer best fits what you want me to report?”

Add a concrete example in your own words. That might be writing, handling something at work or an activity you avoid. Explain what happens and what you do in response, without trying to make the answer sound more or less serious.

Ask how the result fits the assessment

The British Association of Dermatologists2 says the doctor assesses which kind of hyperhidrosis is present and whether an underlying cause needs investigation.2

Ask how the questionnaire result fits with the clinician’s assessment. If you hoped it would settle the entire diagnosis, ask what else the team needs to know. Bring your questions about the pattern of sweating and any medicines you want reviewed.

You could say: “What does this score tell you about my situation, and what still needs assessment?”

A later score can be part of the review

The HDSS instrument describes repeating the questionnaire after treatment as part of evaluating treatment effectiveness and patient satisfaction.1

Ask when it will be repeated and which body area you should consider then. At the review, describe what has changed in the activities that mattered to you at the first appointment. If your answer stays the same but your experience feels different, explain that difference to the clinician.

Before leaving, check what the team wants you to record between visits and when to contact it with concerns. Keep the review date beside those instructions.
