Before returning a home sleep-test kit, check the paperwork your own sleep service requested. Keep those forms with the equipment-return instructions.

Check the handoff

Ask your service which forms need completing and how to return them with the kit. If a question is unclear or you cannot complete a section, tell the team and ask what to do.

Try: “Here are my sleep-test forms. Is anything missing, and who should I contact to discuss the results?”

Keep the result contact

Liverpool Heart and Chest Hospital1 says results are not immediately available after the test; the report goes to the sleep-clinic consultant or referring consultant.1

Follow your own service’s return arrangements. Save the result-contact details with your appointment notes so you can ask about follow-up.

Did I Do That in My Sleep? includes a Visit Brief. Use it to keep sleep-service questions and follow-up details together. See the book.
