For diagnosed functional dyspepsia, a prescriber may discuss a tricyclic antidepressant as a gut–brain neuromodulator, intended to reduce abnormal nerve sensitivity. The British Society of Gastroenterology’s1 2022 guideline supports this drug class as second-line treatment, with a strong recommendation and moderate-quality evidence. The reason for prescribing and the side effects need explanation.1
Who entered the imipramine trial?
The Hong Kong trial randomized 107 adults after symptoms persisted through eight weeks of esomeprazole and four of domperidone. Participants were H. pylori-negative, with normal endoscopy and abdominal ultrasound.2
Treatment was imipramine 25 mg nightly for two weeks, then 50 mg nightly, or placebo, for twelve weeks.2
How many reported relief?
Primary outcome: overall satisfactory relief, a yes/no patient rating.2
| Treatment | Relief at twelve weeks |
|---|---|
| Imipramine | 35/55, or 63.6% (95% confidence interval 50.4–75.1%) |
| Placebo | 19/52, or 36.5% (95% confidence interval 24.8–50.1%) |
Imipramine outperformed placebo (P = .0051). However, 10/55 stopped because of adverse events versus 4/52, including dry mouth, constipation and drowsiness with imipramine.2
What limits that result?
The intention-to-treat analysis carried previous observations forward for missing data. 81 completed follow-up and met adherence criteria. The relief question was unvalidated; this selected, single-center trial does not establish benefit for every upper-abdominal symptom.2
Unfunded; two authors disclosed pharmaceutical-industry relationships.2
What should the prescriber check?
The cited US label covers depression and certain childhood bedwetting, rather than dyspepsia. Digestive treatment is off-label.3
Imipramine is contraindicated with MAO inhibitors and during acute recovery after a heart attack. Review heart disease, urinary retention, narrow-angle glaucoma, seizures, pregnancy and breastfeeding. Other antidepressants and cimetidine, an acid-reducing medicine, can increase imipramine exposure; additional anticholinergic medicines can worsen unwanted effects.3
Its boxed warning concerns increased suicidal thinking and behavior in children, adolescents and young adults during short-term antidepressant studies. Patients of every age need monitoring for new mood or behavior changes, particularly early in treatment or with dose changes. Report such changes promptly.3
The label also warns about low blood sodium, particularly in older adults, people taking diuretics or those who are volume-depleted. Drowsiness can impair driving, and alcohol can increase sedation. Agree with the prescriber how treatment will be reviewed and eventually reduced; do not stop an established prescription abruptly.3
Ask which symptom is the treatment target and what improvement would justify continuing despite dry mouth or constipation. Write down who to contact if unwanted effects occur before the review.
“Time It: Your Belly Clock” in Bloated by Dinner separates meal times, symptoms and medicines into a dated record. For this appointment, mark the previous treatments and their review dates so the clinician can distinguish persistent symptoms from side effects that started after a medicine change.
This article is educational. Research doses describe a trial, not instructions for starting or changing medicine. Treatment needs a confirmed diagnosis and a prescribing clinician.
