Can Lebrikizumab Injections Be Spaced After Eczema Improves?
SkinArticleThe maintenance trial studied selected responders. Current US labeling includes four-week and eight-week options.

A longer interval can mean fewer injections, but it needs a response assessment and a written plan for returning symptoms. The maintenance evidence is specific about who reached that stage.
Who entered the maintenance comparison?
In ADvocate1 and ADvocate2, week-16 lebrikizumab responders without rescue treatment were randomized again to 250 mg every two weeks, every four weeks or placebo withdrawal for another 36 weeks. This was a selected group, not everyone starting treatment.1
What did four-week dosing maintain?
Among participants already achieving at least 75% improvement in the Eczema Area and Severity Index, the estimated proportions maintaining that response at week 52 were:1
| Interval | People | Estimate |
|---|---|---|
| Every four weeks | 115 | 81.7% |
| Every two weeks | 112 | 78.4% |
| Withdrawal | 57 | 66.4% |
These are imputed estimates, not simple observed counts. Rescue topical treatment was permitted and affected the analysis rules. The study did not establish four-week dosing as superior or equivalent to two-week dosing. Dermira, a Lilly subsidiary, funded it.1
Does the current US label also allow eight weeks?
Yes. The September 2026 EBGLYSS label includes 250 mg every four or eight weeks for maintenance, after adequate response at week 16 or later following initial treatment. It covers moderate-to-severe atopic dermatitis in adults and children 12 or older weighing at least 40 kg, when prescription topical treatment does not adequately control disease or is inadvisable.2
The 2023 trial did not randomize eight-week dosing.1 The label bases that interval on models relating drug exposure to response over time. Discuss this evidence separately with your dermatologist.2
What belongs in the follow-up plan?
Ask which signs count as adequate response, what to do if itch or patches return before an injection, and which topical treatments to use. Spacing injections and stopping treatment require different plans.
The label warns about conjunctivitis and keratitis: report new or worsening eye symptoms. Discuss vaccinations before treatment, and avoid live vaccines immediately before or during treatment. Existing helminth infections need treatment before starting.2
The “What to Say at the Appointment” section of Skin, Head to Toe recommends bringing the question and a complete product and medicine history. Add your injection dates and any eye symptoms so the review covers tolerability as well as skin improvement.
Agree any injection interval or treatment change with the prescribing clinician. Trial withdrawal groups are not instructions to stop treatment.
