Lubiprostone for IBS-C: Four Response Checks

Why the original paper and FDA label report different response rates.

  3. Keep the two analyses separate
  4. Plan the safety review

Bring your symptom diary to a prescription review with a specific question: has the treatment changed the problem you most wanted help with? Record pain and bowel changes separately, and include days when symptoms or side effects interrupted your routine.

Check the indication and dose

The current US label specifies women aged 18 or older with IBS-C, at 8 micrograms twice daily. The 24-microgram twice-daily dose belongs to other constipation indications. Liver impairment can require adjustment. Do not increase an IBS-C prescription yourself.1

Read what response meant

Two 12-week trials randomized 1,171 adults; 1,154 entered the efficacy analysis. Response required qualifying relief in at least two of three months.2

A qualifying month meant moderate-or-better relief all four weeks, or significant relief at least two weeks, without moderate/significant worsening, increased rescue use or withdrawal for ineffectiveness. This patient-rated outcome was not a combined pain-and-complete-bowel-movement endpoint.2

Keep the two analyses separate

The paper’s pooled response was 17.9% with lubiprostone versus 10.1% with placebo, carrying previous ratings forward when entries were missing.2

FDA’s3 primary analysis instead assigned missing weekly ratings “unchanged”: 13.8% versus 7.8% in one trial; 12.1% versus 5.7% in the other. These are different analyses of the same trials.3

Sucampo funded the research; authors disclosed company employment, ownership or consulting relationships.2

Most participants did not meet this definition; different endpoints prevent comparisons with other medicines.

Plan the safety review

The label contraindicates known or suspected mechanical bowel obstruction. Taking capsules with food and water may reduce nausea. Stop and contact the prescriber for severe diarrhea or fainting; report chest tightness or difficulty breathing promptly. Discuss pregnancy plans and other medicines before treatment.1

The 2022 AGA4 guideline gives lubiprostone a conditional recommendation with moderate-certainty evidence for IBS-C. Choosing it involves your preferences and the balance of benefit and harm.4

Bring separate notes on pain, stool frequency, stool form, rescue medicines and adverse effects. Ask: “Which changes would make this worthwhile for me, and when will we review them?”

“The Visit Script” in Bloated by Dinner helps organize your symptom pattern, medicine and supplement list, and questions about what has been checked and what comes next. Its tracking schedule does not set a prescription’s duration.
