Miconazole Mouth Tablets: The 10mg Trial
Breath & MouthArticleCompared with itraconazole capsules.

A Chinese trial compared 10mg miconazole mouth-adhering tablets with 100mg itraconazole capsules, each once daily for 14 days.1
The studied tablet
Of 343 randomized people, 340 entered the main analysis. Eligibility excluded HIV and low-saliva conditions, although some ineligible participants remained.1
One miconazole patient developed thrombocytopenic purpura, a serious platelet disorder judged drug-related.1
Cure and uncertainty
Signs and symptoms cleared in 77/170 versus 71/170 at 14 days: difference 3.53 percentage points, 95% confidence interval −7.00 to 14.06. Reported noninferiority allowed a 10-point margin; it did not establish equality.1
The open-label trial was registered after recruitment ended. Protocol and paper describe different missing-outcome rules. The paper reports no specific funding; its registry lists Tigermed as funder.1
The prescription to check
US Oravig contains 50mg miconazole and has its own prescribing instructions: one tablet applied to the upper gum daily for 14 days, without crushing, chewing or deliberately swallowing it. It is contraindicated with miconazole, milk-protein or other ingredient allergy. Stop and contact your clinician for allergy symptoms. The label warns about potentially serious allergic reactions and increased warfarin effects; warfarin users need clotting-test and bleeding monitoring. Other medicine interactions are possible despite low absorption. Animal findings raise fetal-risk concerns; human pregnancy data are unavailable. Discuss pregnancy or breastfeeding with the prescriber.2
IDSA still lists its 2016 guideline as current. It recommends topical options for mild oral candidiasis and fluconazole for moderate-to-severe disease. For disease unresponsive to fluconazole, it recommends itraconazole solution, among other options. That is a different formulation from the trial’s capsule comparator. Denture-related candidiasis also needs denture disinfection alongside antifungal treatment.3
Arrange an assessment for painful or difficult swallowing, or thrush symptoms with a weakened immune system. NHS4 guidance also advises medical review if mouth-gel treatment has not helped after a week. Those are UK gel instructions, not a reason to alter a US tablet prescription.4
Our white-tongue guide discusses other causes of a coating. Is It My Breath? distinguishes ordinary coatings from thrush and persistent patches in “Reading the Tongue.” See the book.
