Bring the ointment tube and your written application schedule to a lichen sclerosus follow-up. Tell the clinician what you currently use, including moisturizer, and show where the instructions become unclear. Ask for a plan that covers both settled disease and symptoms that return between appointments.

Who entered maintenance?

In the Italian study, 27 women started twelve weeks of daily mometasone 0.1% ointment; 25 entered randomized maintenance after improving. The 52-week comparison used twice-weekly mometasone, daily vitamin E oil or daily cold cream. This was maintenance after initial treatment, not a test for undiagnosed itching.1

The 2026 German S3 guideline recommends potent or ultrapotent topical steroids for women with genital lichen sclerosus and identifies mometasone among maintenance options. It recommends emollients alongside standard therapy. Ask which product addresses the inflammation and which provides supporting skin care.2

How certain was protection?

Observed relapses were 0/8 with mometasone, 5/9 with vitamin E and 5/8 with cold cream. One steroid participant was lost to follow-up, so zero observed relapses cannot guarantee protection. Relapse meant at least 5/10 for one symptom, a severe clinical sign or worsened scarring. The small, open-label study used unblinded assessments. It did not establish cure or cancer prevention. The authors reported no funding and no conflicts.1

A symptom change should prompt an examination when your clinician advises one. The guideline says worsening itch, pain or skin findings may require review for other causes, including contact allergy, infection or malignancy, as well as checking whether treatment was used as intended. Do not simply add applications whenever the area feels different.2

What belongs in your prescription plan?

The UK Elocon ointment label lists psoriasis and atopic dermatitis, not lichen sclerosus. It also lists genital itching among contraindications. Specialist treatment of confirmed lichen sclerosus needs a product-specific explanation; this trial is not permission to use leftover eczema ointment for genital itch. Ask the prescriber to explain the indication, application site, amount and off-label prescribing context.3

The label warns about skin irritation, infection and adrenal suppression from systemic absorption. Pregnancy and breastfeeding require individual discussion. Bring concerns about thinning, burning or a possible reaction to the prescriber before changing the schedule yourself.3

The guideline recommends regular review, giving three to six months initially and twelve months once stable as examples. Follow-up should assess symptom control, skin appearance, urinary or sexual difficulties and concerning lesions. Agree your own interval and a route for earlier review.2

Down There, Decoded discusses lichen sclerosus in “The Itch That Isn’t Yeast,” including clinician-directed steroid treatment and regular checks. It distinguishes the observational evidence on long-term treatment from proof that treatment itself caused every difference.

This article is educational and does not replace diagnosis or prescribing advice from your own clinician. Do not start, stop or change a genital steroid regimen based on a trial summary.
