Orlistat and Oily Spotting: What the Side-Effect Figures Mean
Bloating & DigestionArticleOily leakage, urgency and loss of bowel control are separate outcomes in the prescription label. Check the dose studied, your meal pattern and the symptoms that need medical attention.

Oily marks on underwear after starting orlistat can be a medicine effect. Orlistat inhibits digestive lipase enzymes, reducing absorption of some fat from food; that unabsorbed fat leaves in stool. Oily spotting, gas with discharge, urgency and difficulty controlling bowel movements are listed side effects.1
Read the percentages with the dose and timeframe
The US Xenical label summarizes seven double-blind, placebo-controlled trials. Its first-year adverse-event table compares 1,913 people taking Xenical 120 mg three times daily plus diet with 1,466 taking placebo plus diet.2
| Year-one event | Xenical | Placebo |
|---|---|---|
| Oily spotting | 26.6% | 1.3% |
| Bowel urgency | 22.1% | 6.7% |
| Fecal incontinence | 7.7% | 0.9% |
These percentages describe participants reporting each event, rather than the proportion of doses or days affected. They cannot predict how often an individual will have leakage.2
The NHS3 distinguishes prescription Xenical from lower-dose pharmacy products such as Alli and Orlos.3 Do not transfer the 120 mg trial percentages to another dose as though the product and exposure were identical.
Meal fat affects symptoms, but needs a workable plan
MedlinePlus1 advises spreading daily fat, carbohydrate and protein across three main meals. Taking orlistat with a diet supplying more than 30% of calories from fat, or with one very high-fat meal, makes side effects more likely.1
If a meal is missed or contains no fat, NHS3 directions say not to take a capsule for that meal.3 Follow your own packet and prescribed schedule; ask a pharmacist to clarify the instructions if meals are irregular.
Bring a few ordinary meals to the discussion, including sauces, dressings and snacks, rather than trying to solve leakage by cutting out more and more foods. Ask a dietitian or prescriber to translate the dietary advice into meals you can manage at home and work.
Early effects can persist
The label says these digestive events generally first occurred within three months of treatment. About half of recorded episodes lasted less than a week, but some people experienced digestive adverse events over six months or longer.2
Those episode durations do not establish a deadline by which your symptoms must disappear. NHS3 guidance says to contact a pharmacist or doctor about effects that bother you or do not go away.3 Describe missed work, urgent toilet trips and accidents as well as stool appearance.
Check vitamins and symptoms that need prompt care
Orlistat also reduces absorption of some fat-soluble vitamins. MedlinePlus1 advises a daily multivitamin containing A, D, E, K and beta-carotene, taken at least two hours apart from orlistat. Ask your pharmacist to check the product and schedule alongside your other medicines.1
Call your clinician immediately for severe or continuous abdominal pain.1 Rectal bleeding is among the serious effects for which NHS3 guidance advises urgent help.3 The Xenical patient leaflet says to stop taking it and call a doctor right away for liver-warning symptoms such as yellow skin or eyes, dark urine or pale stool.2 Do not treat these as routine oily spotting.
A specific way to ask for help
Try: “I started this dose on this date. I’m getting oil marks, urgency, or bowel accidents this often. Here are my meals and medicine times. Can we review the plan and whether this treatment still suits me?”
The Digestive book includes a visit script built around symptom timing, meals, stool descriptions and a complete medicine list. Bring those notes so the clinician can compare symptom and treatment dates; do not delay urgent care to finish a diary.
General education for adults already discussing or using orlistat, not a recommendation to start a weight-loss medicine or a personal diet prescription. Numerical side-effect rates below concern prescription Xenical 120 mg trial data, not every orlistat product. US labeling and UK NHS guidance are identified. Discuss bothersome effects and treatment changes with your prescriber or pharmacist.
Girl Playbook sells this book. It provides education and worksheets, not individualized medical care.
