A Patency Capsule: Why the Dummy Test Comes Before the Camera
Bloating & DigestionArticleUnderstand the passage check, the team’s decision and the safety instructions before capsule testing.

If you have swallowed a patency capsule and develop nausea, vomiting, severe abdominal pain or swelling, or severe chest pain, get medical help immediately through an emergency department or GP, as Kingston and Richmond advises. Do not wait for the capsule to dissolve or for a planned passage check.1
First: checking the route
A patency capsule checks whether a video capsule can pass through the bowel. It breaks down into smaller pieces. Guy’s and St Thomas’ describes using it when small-bowel narrowing is suspected, including in people with previous small-bowel surgery, known narrowing or a history of obstruction.2
Ask which part of your history led to this recommendation. You can say, “What concern are you checking before the camera test, and what would change the plan?”
Tell the unit if swallowing pills is difficult or you have a known narrowing of the food pipe. The trust asks patients to report these before the procedure.2
Second: taking pictures
The video capsule has a camera; the patency capsule is a preliminary passage check. If a camera capsule becomes stuck in the small bowel, an endoscopy or surgery may be needed to remove it.2
Ask what the camera examination is intended to investigate in your case. Keep that question separate from the passage-check question. Understanding why you need images does not remove the need to discuss how the team assesses passage.
The team checks what happened
Guy’s and St Thomas’ describes an X-ray or CT scan to locate the patency capsule after swallowing. If it has been held up, the team discusses other tests instead of proceeding with video capsule endoscopy.2
Confirm your own unit’s checking arrangements before booking work or transport around the appointment. Ask what to report if you do not notice the capsule passing, who reviews the check and how you will hear the decision about the camera test.
Do not decide that the next step is cleared because you feel well or think the capsule has passed.
Keep the safety instructions with the appointment
Kingston and Richmond says to avoid powerful magnets, including an MRI scanner, until the patency capsule has passed. Tell any MRI team about the capsule and have your clinical team confirm passage before a scan.1
Ask your unit for current instructions about preparation, medicines and eating afterward. Have it clarify conflicting directions before changing anything. Get a contact route for urgent concerns and use it if symptoms occur.
