Pilocarpine Drops for Dry Mouth: Reading the Frailty Trial

The primary result, smaller-than-planned sample and practical care questions.

For someone already managing several medicines, adding drops for dry mouth needs a clear goal and a review date. The question is whether the person feels and functions better, as well as what the extra treatment adds to daily care.

Who took part?

The 2026 Dutch trial1 randomized 92 adults with frailty and/or a life-limiting condition, and dry-mouth severity at least 5/10. 44 received pilocarpine drops and 48 placebo for four weeks. Recruitment was below the 120 participants required by its sample-size calculation.1

Did the drops beat placebo on the main outcome?

At week four, the adjusted between-group dryness-score difference was −0.37 points, with a 95% confidence interval from −1.31 to +0.58. It included no difference. The underpowered trial did not establish superiority or prove no benefit.1

Oral-health quality of life favored pilocarpine at two weeks, a secondary outcome. That does not replace the primary four-week dryness result. ZonMw funded the study.1

Does this settle every use of pilocarpine?

These findings concern frailty or life-limiting illness, not a menopause-specific population.1

Our pilocarpine and cevimeline tablet guide discusses those separate prescription products. If drops are proposed, ask for their name, concentration, route, intended benefit and review point in writing. A medicine name alone does not specify the preparation.

What mouth care still needs attention?

NIDCR’s caregiver guidance says dry mouth can make dentures rub and cause sores; it also increases risks of tooth decay and fungal infection. It advises brushing gently with fluoride toothpaste at least twice daily, regular flossing and dental visits even when no natural teeth remain. Sores, pain, swelling or bleeding gums need prompt dental attention.2

It also recommends rinsing with water after inhalers or syrup-based medicines. If medicines contribute to dryness, dose changes or substitutions belong with the prescriber. Artificial saliva may be another option. Dry mouth itself is not an inevitable part of aging.2

For a care review, bring the actual medicine list, including liquids and inhalers. Add the specific difficulty: dentures hurt at lunch, food sticks, speaking is uncomfortable or symptoms interrupt sleep. That is more useful than asking whether a product is “good for dry mouth.”

What should go in the record?

Write the start date, the problem you want to improve and what changed afterward. If another medicine changed at the same time, put both dates on the page. A short record can help the team judge the plan without relying on a remembered good day.

“The Medicine List” and “Supporting Saliva” in Is It My Breath? connect dryness with the full prescription and nonprescription list and distinguish comfort measures from supervised saliva-stimulating treatment. Use those sections to organize the discussion, with the care team’s instructions setting the treatment schedule.

Ask the treating team about the exact formulation and route. This research summary is not a prescription or instructions for preparing drops at home.
