Before adding a medicine for persistent upper-abdominal symptoms, bring your previous test results and a list of treatments tried. Note which symptoms still interfere with meals. These checks can help organize the next appointment.

Who was included?

In Thailand, 72 adults with functional dyspepsia despite at least eight weeks of acid suppression received pinaverium or placebo alongside omeprazole for eight weeks. Endoscopy was normal; Helicobacter pylori testing was negative. IBS and reflux disease were excluded.1

Did relief improve?

The main outcome, adequate relief on more than half of days, did not differ significantly at week eight (P = .363). That does not establish equivalence. Published response denominators conflict with the participant flow; missing-data handling is unclear. An isolated secondary early-satiety finding does not settle efficacy.1

Who stopped treatment?

Four pinaverium recipients stopped because of abdominal pain or epigastric burning. Only 32 of 36 active-treatment participants and 34 of 36 controls completed follow-up. This small, short study cannot establish uncommon harms or durable benefit.1

What guides prescribing?

Health Canada2 lists prescription Dicetel 50 mg tablets as marketed and links a December 2014 monograph. That Canadian record does not establish availability elsewhere.3

The Canadian monograph lists IBS symptoms and functional biliary disorders, not functional dyspepsia. Using it for dyspepsia falls outside those listed indications. Tablets can injure the esophagus: the label specifies swallowing them whole with a glass of water during meals, never while lying down or just before bed. Existing esophageal injury or a hiatus hernia needs particular attention. It advises against use during breastfeeding; pregnancy requires the prescriber’s assessment. Do not copy a research regimen or assume another formulation has the same instructions.2

The 2022 British Society of Gastroenterology guideline supports specialist referral when the diagnosis is uncertain or symptoms are severe or persist despite first-line treatment. Care can include dietetic support and gut-brain behavioral therapies. For severe or refractory symptoms with weight loss or food restriction, it recommends assessment for disordered eating and early dietitian involvement. Bring new weight loss or a shrinking food list to the appointment instead of adding restrictions yourself.4

Bloated by Dinner provides a “Visit Script”: describe your symptom pattern, list medicines and supplements, and ask what has been ruled out and what comes next. Bring those answers back when discussing whether another drug has enough evidence for your particular symptoms.
