Prescribed Pivya?Flag Valproate and Methotrexate
Bladder & Pelvic FloorArticleThe U.S. pivmecillinam label names two combinations that need a prescriber’s review. Give the pharmacist your existing medicine details before starting.

If you are prescribed Pivya (pivmecillinam) for a bladder infection and take valproate or methotrexate, contact the dispensing pharmacist or prescriber before starting it. The current U.S. label says to avoid concurrent valproate treatment and to consider an alternative where possible with methotrexate.1
Valproate: ask about the carnitine warning
The Pivya label warns that concurrent valproic acid or valproate can increase the risk of carnitine depletion. If the combination is necessary, it calls for counselling about symptoms including low blood sugar, muscle aches, fatigue and confusion.1
The label also says no clinical effects from decreased carnitine have been associated with short-term Pivya treatment, while the effects of repeated short courses on carnitine concentrations are unknown.1 That uncertainty is a reason for a prescription review, rather than assuming a short course removes the interaction warning.
MedlinePlus2 explains that divalproex sodium, valproate sodium and valproic acid are similar medicines used by the body as valproic acid. Bring the pack if you are unsure which name appears on your record.2
Do not stop valproate yourself. MedlinePlus2 warns that suddenly stopping it can cause a severe, prolonged and potentially life-threatening seizure.2 Ask the clinician to choose the infection treatment and explain what to do with your established prescription.
Methotrexate: include the next dose date
Pivya’s prescribing information says penicillin-class drugs, including Pivya, can reduce methotrexate clearance from the body. It advises considering an alternative therapy where possible.1
The NHS Specialist Pharmacy Service3 explains that penicillins can increase methotrexate toxicity risk. Its guidance for low-dose methotrexate allows cautious use of a short penicillin course in some circumstances, with advice to watch for toxicity.3 That general guidance does not replace an individual check of the U.S. Pivya prescription.
Tell the pharmacist when you last took methotrexate and when the next dose is due. Ask who will coordinate with the clinician managing it, whether the antibiotic should change and whether any monitoring is needed.
Report a sore throat, mouth ulcers, or unexplained bruising or bleeding promptly; the Specialist Pharmacy Service identifies these as possible methotrexate toxicity signs.3 Do not change your methotrexate schedule on your own.
If you have already taken both
Call the pharmacist or prescriber promptly with the names, amounts and times taken, plus any new symptoms. Ask for instructions before the next doses. Do not add a carnitine supplement as a home correction or substitute leftover antibiotics.
Keep the infection assessment moving while the medicines are checked. Seek care right away for possible kidney-infection symptoms, such as fever and chills, back or side pain, nausea or vomiting with urinary symptoms. NIDDK advises immediate care for kidney-infection symptoms because the infection can lead to sepsis.4
The “What to Say in the Room” section in Laugh, Sneeze, Jump asks readers to bring every medicine, including products bought without a prescription. Add the new antibiotic to that list and leave the review with written instructions for both the infection and your ongoing treatment.
Educational information about the U.S. Pivya label. A pharmacist or prescriber should check the whole medicine list and arrange an individual treatment plan. Do not stop established medicines to make room for an antibiotic.
