Plecanatide for IBS-C: Reading the Combined Response
Bloating & DigestionArticlePain and bowel improvement had to coincide.

The American Gastroenterological Association’s1 2022 IBS-C guideline conditionally suggests plecanatide, with moderate certainty of evidence. Its explanation says improvement may be small for some people. A conditional recommendation means individual risks and preferences belong in the prescribing decision; it does not select a medicine for everyone with constipation.1
The response threshold
Two 12-week trials tested 3 mg, 6 mg or placebo. The 2023 pooled efficacy groups included 724 adults taking 3 mg and 729 taking placebo.2
Response required at least 30% less weekly-average worst daily abdominal pain plus at least one extra complete spontaneous bowel movement, in the same week for at least six of 12 weeks. “Complete spontaneous” means without laxative use and with a sense of complete emptying.23
That distinction gives a follow-up appointment two questions: did the pain change, and did bowel movements change? Record both, including the feeling of incomplete emptying. A frequency count alone cannot describe the second part fully, and the trial threshold required the pain change too.
Reanalysis limits
Across all three arms, 1,857/2,176 completed; efficacy analyses excluded duplicates. The report does not detail missing-diary handling. Synergy Pharmaceuticals funded trials; Salix funded reanalysis; authors included employees.2
This reused trial data; response did not mean symptom-free remission.2
Keep our IBS-C prescription overview beside this narrower result. The guideline covers several medicines. These trials did not compare plecanatide with them.
Diarrhea changes the review
Diarrhea occurred in 4.3% taking 3 mg versus 1.0% taking placebo.2
The US Trulance label specifies 3 mg once daily for adults with IBS-C, with or without food. Stop taking it and call the doctor if severe diarrhea develops; prescribing instructions call for rehydration. Do not continue through severe diarrhea to reach a study assessment date.3
The boxed warning concerns serious dehydration in children. Trulance is contraindicated below age six; the label says to avoid it from six to under 18. It is also contraindicated with known or suspected mechanical gastrointestinal obstruction. Keep it securely out of children’s reach.3
If a dose is missed, skip it and take the next dose at the regular time rather than doubling up. Keep tablets in the original bottle, protected from moisture, with the desiccant in place. Discuss pregnancy or breastfeeding and the full medicine and supplement list with the prescriber.3
Record both symptoms
Before starting, ask the prescriber which changes you should record and when to review them. A useful entry separates pain intensity, bowel movements, stool form, complete or incomplete emptying, rescue laxatives and diarrhea. Include the dates of prescription changes. Those details let you describe a mixed result, such as more bowel movements with pain that still interrupts meals.
Bring a question tied to that record: “My bowel frequency changed, but the pain still limits dinner. Here are the dates, stool pattern and rescue medicines. Which symptom are we trying to improve next, and what would change the prescription plan?”
Bloated by Dinner includes “Week 1: Start the Clock” and “The Visit Script.” They organize pain, bowel movements, Bristol stool types, medicines and the questions about what has been checked and what comes next. Use that preparation for the review; the book’s logging schedule does not set a prescription’s duration.
