Tenapanor Response Timing: Bowel Changes, Pain and Bloating

How to read the study's response thresholds and plan a treatment review.

If a prescription changes your bowel pattern before your evening bloating improves, bring both observations to the review. A diary that says only “better” or “not working” can lose the distinction between more bowel movements and less discomfort.

What the timing analysis measured

A 2026 post hoc analysis pooled 1,372 adults with IBS-C from three trials; 82.4% were women, with an overall mean age of 45.3. Median time to a first bowel-frequency response was two weeks with tenapanor versus four with placebo; first bloating response took five versus eight weeks, and first pain response four versus six.1

“Response” meant at least one additional complete spontaneous bowel movement per week, or at least a 30% reduction in the weekly abdominal symptom score. “Sustained” meant two consecutive responding weeks, not lasting remission.1

This was an exploratory, non-prespecified pooled analysis, with no adjustment for multiple statistical comparisons. Ardelyx funded it. Group timing estimates do not give an individual deadline or establish superiority over another prescription.1

What counts as a bowel response?

The US IBSRELA label defines a complete spontaneous bowel movement as one occurring without laxative use and accompanied by a feeling of complete emptying. The medicine is indicated for IBS with constipation in adults.2

For your record, separate the number of bowel movements from the feeling afterward. “Went twice but still felt unfinished” communicates something that a frequency count alone misses. Keep pain, bloating and stool form on separate lines, with dates beside changes in medicines or rescue treatments.

Our IBS-C prescription overview covers the wider treatment discussion. A timing analysis of one medicine does not choose among those options for you.

When diarrhea changes the plan

In the label’s 26-week trial, diarrhea occurred in 16% on IBSRELA versus 4% on placebo. Across the two controlled trials, severe diarrhea occurred in 2.5% versus 0.2%. The Medication Guide says to stop taking IBSRELA and call the doctor if severe diarrhea develops; the prescribing information calls for suspending treatment and rehydration.2

It contraindicates use with known or suspected mechanical gastrointestinal obstruction. It instructs taking the medicine immediately before the first meal and before dinner, and skipping a missed dose rather than doubling up. Review other medicines with the prescriber or pharmacist.2

A published response timeline is no reason to endure severe diarrhea while waiting for a later week to arrive.

Give the review a specific question

Bring a dated note: “My bowel frequency changed, but bloating still interrupts dinner. Here is the stool pattern, the symptom record and any diarrhea. What improvement are we looking for, and when should we reassess?”

“Week 1: Start the Clock” in Bloated by Dinner records meal timing, bloating, gas, pain, urgency, Bristol stool types and medicines. “The Visit Script” adds the full medicine and supplement list and asks what has been checked and what comes next. Use those sections to prepare the review; the book’s tracking schedule does not set a prescription’s duration.

Agree on a review date with your prescriber. Study timing is not an instruction to continue through severe adverse effects or change a dose yourself.
