Heavy periods with von Willebrand disease: four VWDMin trial checks

What a factor-infusion comparison adds to a specialist conversation about persistent menstrual bleeding.

A woman in her early forties reviews a period record with a hematology clinician at a consultation table.

If heavy periods continue after a von Willebrand disease diagnosis, the next appointment needs more than a request for something stronger. Ask which bleeding outcome another treatment should improve and what extra appointments or daily doses it would require.

Check who was studied

VWDMin enrolled 39 participants with mild or moderate von Willebrand disease and heavy periods; 36 completed it. Eligible ages were 13–45. Severe disease and older women were not studied.12

Check the infusion schedule

Participants tried both treatments in randomized order, for two cycles each: recombinant von Willebrand factor 40 IU/kg intravenously on day 1, or tranexamic acid 1300 mg three times daily on days 1–5.12

Check the unit behind the result

The adjusted difference favored tranexamic acid by 46 bleeding-chart points (95% confidence interval 2–90), not milliliters. Neither treatment normalized bleeding-chart scores.1

Check the limits before changing treatment

The open-label trial stopped early for slow recruitment. It did not establish superior bleeding control with this factor-infusion regimen or settle other doses and schedules.12

Persistent bleeding still needs a broader review

The 2021 ASH3/ISTH/NHF/WFH guideline encourages joint gynecology and hematology care where feasible. It recommends regular assessment and treatment for iron deficiency or anemia. A known bleeding disorder also does not remove the need to assess gynecologic causes such as fibroids or polyps, especially when initial treatment has not controlled bleeding.3

Its menstrual-treatment recommendations are conditional and based on very low-certainty evidence. Pregnancy intentions affect the options discussed: the guideline favors tranexamic acid over desmopressin for women wishing to conceive, and either hormonal treatment or tranexamic acid over desmopressin for those not wishing to conceive. Those recommendations predate VWDMin.3

Tranexamic acid still needs a suitability review. The NHS4 asks patients to disclose previous blood clots, kidney problems, epilepsy, pregnancy or plans to conceive, and irregular bleeding.4 Our tranexamic acid prescribing questions cover the medicine discussion in more detail.

Bring your treatment dates and the bleeding that remained: overnight changes, flooding, days missed from work, and fatigue. Ask: “What will we change if the next two cycles remain this heavy, and which team will arrange the follow-up?”

Period, Handled includes a Period Scorecard and a section on von Willebrand disease. Its record covers product changes, night changes, clots and tiredness. Keep those observations beside your prescribed treatment dates for the review.

For education, not a treatment or infusion plan. Do not change prescribed bleeding-disorder treatment without your specialist.
