Planning a journey around toilet stops is tiring enough without finding out that you collected the wrong sample. If bile acid diarrhea is being considered, ask which test has been ordered before you start preparing.

The collection difference

Test approachWhat to clarify before collection
Fasting serum C4The blood-test preparation and any medicines or liver conditions that affect interpretation. 1
48-hour fecal bile acid testingThe collection period, prescribed diet, storage and delivery instructions for that laboratory. 2
Combined C4 and random stool testingWhether this combined panel is available to you, and how to coordinate both samples. 2

Mayo’s combined approach uses a fasting C4 blood sample plus a random stool sample. It avoids the 48-hour stool collection but still requires both kinds of sample. Availability and instructions depend on the service arranging your test.2

Four questions for the order

  1. Which exact test name should appear on the request?
  2. Where do I get the complete collection instructions?
  3. Who do I contact if collection or transport goes wrong?
  4. What will the clinician do with a positive, negative or uncertain result?

Ask these before changing your food intake to prepare. Keep the instructions with the collection kit so you can check the steps when you need them.
