4 Checks on Yearlong Budesonide for Collagenous Colitis
Bloating & DigestionArticleThe maintenance trial enrolled prior responders and tracked relapse after treatment ended.

In this article
The yearlong budesonide trial studied keeping established collagenous colitis controlled after an initial response.
Confirm who entered maintenance
After eight weeks of open-label induction, 92 adults with biopsy-confirmed collagenous colitis in remission were randomized; 78 were women. Budenofalk doses alternated 6 mg and 3 mg daily, averaging 4.5 mg/day. Dr Falk funded the trial and participated in design and analysis.1
Ask the clinician to confirm the biopsy subtype and what the initial treatment achieved.
Read the remission definition
Clinical remission meant weekly averages below three stools daily, including below one watery stool daily. Throughout 12 months, 27/44 (61.4%) maintained remission with budesonide versus 8/48 (16.7%) with placebo. The reported difference was 44.5 percentage points (95% CI 26.9–62.7).1
Keep both the daily stool count and consistency in the record you bring to review. Ask how urgency, leakage and restrictions on leaving home will be assessed alongside those counts. A numerical threshold does not describe every difficulty that matters to you.
Discuss what happens after stopping
After tapering, 23/28 former budesonide recipients relapsed during selected six-month treatment-free follow-up. That subset is not the full randomized population, and remission during treatment does not establish a cure.1
Agree when to contact the team if watery stools return, what information to record and who will decide whether further treatment is appropriate. Request the taper and review schedule in writing.
Check formulation and monitoring
The UK Budenofalk 3 mg capsule label includes microscopic colitis maintenance for frequently recurring symptoms after successful induction. It advises the lowest effective dose and regular review, no later than 12 months; extension requires an individual benefit–risk assessment.2
US labeling for Northstar budesonide delayed-release capsules covers Crohn’s disease, so microscopic-colitis treatment with that product is off-label. Confirm the exact product and instructions with your prescriber; the shared ingredient does not identify the formulation tested here.3
European guidance strongly recommends budesonide maintenance for collagenous colitis, with moderate-quality evidence. It separately advises individualized consideration of calcium/vitamin D and bone-density monitoring during long-term treatment. The UK label also warns about adrenal suppression, bone effects and interactions including grapefruit juice.24
Bring your medicine list and mention osteoporosis, eye disease, liver problems or previous steroid treatment. Ask which checks apply to you and how the team will decide whether continued treatment remains worthwhile.
Bloated by Dinner includes “The Visit Script,” with a stool-pattern record, a medicine-and-supplement list and questions about what has been ruled out and what comes next.
