Linaclotide for Bloating: Four Constipation Trial Checks
Bloating & DigestionArticleBowel response and bloating relief differ.

In this article
If constipation treatment comes up because your belly feels persistently bloated, ask the prescriber what improvement you should track. A bowel-frequency target and a bloating score answer different questions. One linaclotide trial helps show why keeping both on the page matters.
Check who was included
Participants had chronic idiopathic constipation, bloating of at least 5/10 and no IBS. Of 487 randomized, 483 entered efficacy analysis; 76% completed.1
That selection matters when discussing the result. Tell your clinician what you mean by bloating: pressure, fullness, a visible change or several sensations together. Describe stool frequency, straining and whether you feel emptied afterward, rather than assuming that every bloated abdomen has the same treatment.
US AGA and ACG2 guidance recommends linaclotide for adults with chronic idiopathic constipation who have not responded to over-the-counter treatments.2 Bring the names, doses and duration of anything already tried so that discussion starts from your treatment history.
Check the bowel target
Primary response at 145 micrograms daily was 24/153 versus 13/171 on placebo: at least three complete spontaneous bowel movements weekly, at least one above baseline, in at least nine of twelve weeks.1
Complete meant feeling emptied; spontaneous excluded rescue laxatives in the prior 24 hours.1
Ask which part of your own bowel pattern needs to change and how your clinician wants you to record it. A weekly count without a note about incomplete emptying could miss the symptom that brought you to the appointment.
Check what bloating improvement measured
Secondary mean bloating improvement over 12 weeks was 34.9% at 145 micrograms versus 22.7% on placebo. Girth was not measured.1
Symptom relief does not establish a smaller waist. Ask for a review that considers both the discomfort you report and the bowel changes you record.
Forest and Ironwood funded the study and helped design, analyze and publish it.1
Check the dose and diarrhea plan
Diarrhea occurred in 9/153 at 145 micrograms, 27/160 at 290 micrograms and 4/173 on placebo.1
Current US labeling recommends 145 micrograms daily for adult chronic idiopathic constipation, with 72 micrograms an option according to presentation or tolerability. The adult IBS-C dose is 290 micrograms.3 The trial’s higher dose is not a reason to increase a constipation prescription yourself.
The label contraindicates use with known or suspected mechanical gastrointestinal obstruction. Severe diarrhea requires suspending treatment and rehydration; contact the prescriber promptly if it occurs. Take Linzess on an empty stomach at least 30 minutes before a meal.3 Ask which other laxatives to continue and what to do if symptoms change.
