Colesevelam for Bile Acid Diarrhea: Four SINBAD Trial Checks

The remission result came from a small blood-test subgroup and a short treatment period.

If colesevelam has been proposed for persistent watery stools, take your test report to the treatment discussion. The headline percentage is easier to interpret when you know which participants it describes and what counted as success.

Find the subgroup behind the percentage

SINBAD randomized 168 adults at four Danish centers without inflammatory bowel disease; 41 with C4 blood levels above 46 ng/mL formed the primary subgroup.1

Ask whether your diagnosis rests on that blood test, a SeHCAT scan or another assessment. Our bile acid diarrhea testing guide helps prepare questions about the sample order. The clinician can explain how your result affects the choice of treatment.

Read the counts beside the estimate

Remission occurred in 14/22 on colesevelam versus 3/19 on placebo: adjusted odds ratio 9.1, broad 95% confidence interval 1.9–62.8.1

The secondary SeHCAT analysis came from this same trial.1

Ask for the expected benefit in words you can use: how many bowel movements, which stool forms and how much disruption to the workday. An odds ratio does not tell you your own probability of responding.

Check what remission and duration mean

Remission meant daily averages below three stools and below one Bristol type 6–7 stool during treatment days 6–12, allowing some loose stools.12

Treatment lasted 12 days, with dose adjustment during the first five.1 This comparison cannot establish years of symptom control. Ask when to review your diary, how any dose change will be supervised and what happens if improvement fades.

Nurses supervised titration.2 Follow the prescription you receive, including its preparation and meal instructions, rather than copying a research regimen.

Review safety and the prescribing plan

No serious adverse events were reported; bloating, abdominal pain and nausea occurred.1

The US label lists cholesterol lowering and type 2 diabetes indications; bile acid diarrhea use is off-label. It contraindicates treatment with triglycerides above 500 mg/dL, previous triglyceride-induced pancreatitis or a history of bowel obstruction.3

Ask the pharmacist to check your full medicine list. The US label specifies taking interacting medicines, including levothyroxine and certain combined contraceptives, four hours before colesevelam. It advises stopping and seeking medical attention for severe abdominal pain or severe constipation.3

A local NHS policy approved in October 2025 recommends specialist assessment, with colestyramine first and colesevelam if it is not tolerated.4 That is a regional prescribing pathway, so ask how your own service makes the decision. Our medicine-schedule conversation guide provides wording for that appointment.

Bloated by Dinner includes “The Visit Script”: bring your bowel log and medicine list, describe the pattern, and ask what has been ruled out and what comes next. See the book.
