If a sudden jolt interrupts the moment you are drifting off, you may find yourself waiting for it the next night. Describe what happens and when, especially if it is making bedtime difficult.

What does “sleep start” mean?

Royal Papworth describes sleep starts, also called hypnic jerks, as normal, very common movements occurring just as someone falls asleep. The whole body or the legs may jerk, sometimes with a falling sensation, loud noise or flash of light. This description does not establish the cause of every nighttime movement.1

Note whether you were drifting off, already asleep or fully awake. Use your own words for the sensation rather than trying to reproduce a medical description.

What may make the described pattern worse?

Royal Papworth says sleep deprivation, stress and caffeine-containing drinks may worsen sleep starts. Those possibilities do not prove that a particular drink or stressful day explains your episode.1

If you want to discuss a pattern, write down the sleep you had beforehand and when you had caffeine. Include nights when nothing happened. Ask which observations would be useful before deciding what to change.

When does the timing need a closer look?

Royal Papworth emphasizes accurately identifying unusual nighttime events. The movements, accompanying sensations or dreams, recollection, time of night and possible triggers can help establish the cause. Some people need hospital polysomnography to clarify events; it is not a required test for every jolt.1

Seek an assessment if you are unsure what the episodes are or they are disrupting sleep. Do not label repeated movements throughout the night as sleep starts merely because you recognize the word “jerk.”

Bring what you remember and, if someone has witnessed an episode, their description with their agreement. Keep recollections separate: “I felt a jolt while drifting off” and “My partner saw movement later” may be two different observations for the clinician to consider.

What should I ask at the appointment?

You can use these questions:

  • “Does the timing fit a sleep start, or do you need to consider another explanation?”
  • “Which details should I record, and for how long?”
  • “Would any test help answer the remaining question?”
  • “What should prompt me to contact you again?”

Keep any agreed advice with your notes. Do not change prescribed medicines based on this article; take questions about them to the prescriber.

Did I Do That in My Sleep? includes a 14-Night Lights-Out Record and a Visit Brief. Use them to organize observations and the questions you want answered alongside the clinician’s instructions. See the book.
