Does Lansoprazole Help Persistent Throat Symptoms?
Bloating & DigestionArticleTOPPITS found no advantage over placebo.

Persistent throat-clearing or a lump sensation can interrupt conversations. They do not establish reflux. ACG recommends assessing other causes before attributing symptoms to gastroesophageal reflux disease (GERD).1
Who entered TOPPITS?
346 adults at eight UK ENT clinics had unexplained throat symptoms for at least six weeks. They were randomized to lansoprazole 30 mg twice daily or placebo for 16 weeks, without requiring reflux-test confirmation. NIHR2 funded it; authors declared no competing interests.2
What did it find?
The primary Reflux Symptom Index runs from 0 to 45; higher scores mean worse symptoms. At 16 weeks, the adjusted lansoprazole-minus-placebo difference was 1.9 points (95% CI −0.3 to 4.2; P = .096): no demonstrated advantage.2
Only 220 participants with usable scores at 14–20 weeks entered the primary analysis; 267 had usable follow-up scores at any time. Analyses used complete cases rather than imputing missing outcomes. Improvement in both groups does not establish a drug effect.2
What should a review cover?
ACG recommends reflux testing before proton pump inhibitor (PPI) treatment when throat symptoms occur without heartburn or regurgitation. When both typical and throat symptoms occur, it conditionally suggests considering an 8–12-week twice-daily PPI trial before additional testing. Ask which situation applies; TOPPITS does not rule out benefit for confirmed GERD.1
The current US lansoprazole capsule label includes GERD and erosive esophagitis among its indications, not unexplained throat symptoms. The study regimen is not a self-treatment schedule. Rilpivirine-containing medicines are contraindicated with lansoprazole, as is known hypersensitivity to the formulation.3
The label warns about acute kidney inflammation, potentially serious skin reactions and diarrhea associated with C. difficile. Longer use also warrants review of magnesium, vitamin B12 and fracture risks. Ask the prescriber to check other medicines, including high-dose methotrexate. Do not change an existing prescription solely because this throat-symptom trial was negative.3
Difficulty swallowing, unexplained weight loss or gastrointestinal bleeding needs prompt assessment. ACG recommends endoscopy as soon as feasible when alarm symptoms accompany suspected GERD.1
Bring your symptom timeline, any heartburn or regurgitation, and the prescription’s purpose to review. “The Red-Flag List” in Bloated by Dinner includes swallowing difficulty among reasons to seek care.1
