Nemolizumab for Prurigo Nodularis: Itch Relief and Skin Clearance Are Different Results

How to read the 16-week trial, the US label and the newer long-term follow-up.

If a dermatologist suggests nemolizumab for diagnosed prurigo nodularis, ask what improvement you will track: less itching, fewer nodules, or both. The OLYMPIA 2 trial measured itch and skin changes separately.1

Our nodular-prurigo review notes can help you prepare the symptom history for that discussion.

What did the placebo-controlled trial show?

OLYMPIA 2 randomized 274 adults with moderate-to-severe disease: 183 received nemolizumab and 91 placebo for 16 weeks. The published report found a reduction of at least four points on the 0–10 peak-itch scale in 56.3% versus 20.9%. That measures improvement, rather than absence of itching.1

The skin outcome required a clinician rating of clear or almost clear, with at least a two-point improvement. 37.7% versus 11.0% reached it. An itch response therefore cannot be substituted for a skin-clearance result. Galderma funded the trial.1

Why does the US label give a different itch percentage?

The current US label reports 49% versus 16% for the same trial’s four-point itch improvement. Its analysis counts missing diaries and rescue treatment as nonresponse.2

The FDA review explains the discrepancy: the published analysis allowed extensions beyond the usual seven-day diary windows when entries were insufficient. The analysis without those extensions produced the label’s lower percentages. Both analyses favored nemolizumab over placebo. Compare figures from the same analysis when discussing the expected benefit.3

Does the 100-week follow-up establish lasting control?

The newer open-label extension included 508 participants from earlier trials, with everyone receiving nemolizumab. At week 100, 133 of 144 evaluable participants (92%) achieved the four-point itch improvement. That denominator is essential: it was not 92% of everyone who entered.4

Diary data were substantially missing, and the main analysis used observed results without filling in missing values; it included observations after rescue therapy. The study lacked a placebo comparison. It supports continued-treatment follow-up, rather than proving what happens after stopping. Galderma sponsored it, and several authors were company employees.4

What belongs in a treatment conversation?

In the United States, nemolizumab is approved for adults with prurigo nodularis. The two phase 3 trials enrolled people with at least 20 nodules and severe itch; they do not supply a response estimate for every itchy rash. The label lists headache and eczema-type reactions among common adverse reactions in prurigo nodularis. It also warns about hypersensitivity and advises completing appropriate vaccinations before treatment and avoiding live vaccines during it.2

Ask: “Which itch score and skin findings will we record before starting? When will we review each? What would make us change the plan?” Keep those agreed measures in your appointment notes so a statement such as “better” has a specific meaning.

The Skin Sort in Skin, Head to Toe helps describe where bumps occur, their texture and whether they itch or hurt. Those descriptions can support the visit; the book does not cover this medicine’s trial results.
