Remibrutinib for Chronic Hives Four Evidence Checks
SkinArticleThe diagnosis, response threshold, comparison treatment and bleeding discussion behind Rhapsido.

If antihistamines have left you with persistent hives, a discussion about remibrutinib, sold as Rhapsido, should begin with the diagnosis and the outcome you want to change. These four checks focus on the REMIX trials in chronic spontaneous urticaria, CSU.1
Confirm which type of hives is being treated
The US label revised October 2026 covers adults with CSU or symptomatic dermographism who remain symptomatic despite H1 antihistamines. Its dose is 25 mg twice daily.2
The CSU results below do not describe the separate dermographism trial. Ask your clinician to name the type recorded in your notes before applying a response percentage to it.
For general symptoms and assessment, see our hives guide. Hives with breathing or swallowing problems, or feeling faint, need emergency care.3
Separate low symptom scores from no itch or hives
UAS7 combines a week’s itch and hive severity on a 0–42 scale. At week 12, UAS7 of six or less occurred in 49.8% versus 24.8% with remibrutinib versus placebo in REMIX-1, and 46.8% versus 19.6% in REMIX-2.1
UAS7 of zero was less common: 31.1% versus 10.5%, and 27.9% versus 6.5%, respectively. Zero meant no itch or hives during that scoring week. It did not establish a permanent cure. Novartis2 funded the trials.1
Ask whether a quoted “response” means fewer symptoms, a low score or a score of zero. The threshold changes what the percentage tells you.
Keep the background treatment in the comparison
Participants continued their background antihistamines. The comparison tested adding remibrutinib against adding placebo; it does not tell you to stop your antihistamine. The FDA’s4 controlled efficacy population included 912 adults, with women making up 68% in REMIX-1 and 65% in REMIX-2.4
Write down the antihistamine name, dose and schedule you have actually used, including missed doses and side effects. Ask what would continue if another prescription were added, and when the plan would be reviewed.
Discuss bleeding before treatment and procedures
In the pooled 24-week CSU safety data, bleeding reactions occurred in 9% with Rhapsido versus 2% with placebo. The label advises monitoring bleeding and considering additional risk with antithrombotic medicines. It specifies a 3–7-day interruption before and after surgery or invasive procedures, depending on the procedure and bleeding risk. Live vaccines should be avoided during treatment.2
Ask the prescriber to coordinate any interruption with the procedure team. Bring your complete medicine list and upcoming procedure or vaccination dates to that conversation.
Bring a description the clinician can use
“Describe It Before You Sort It” in Skin, Head to Toe organizes a skin concern by location, appearance and sensation, speed of onset, and accompanying body changes. Add dated photos and your treatment history to those observations so the appointment can address what is still happening between visits.
Discuss prescription treatment with the clinician managing your hives. Study results do not determine which medicine is suitable for you.
