Odor & Sweat
Personal impact boardExcessive sweating appointment worksheet: daily-life impact board
Prepare concrete examples of how sweating affects your day. Organize observations, workarounds and goals into a free printable appointment worksheet.
Use your own examples, whether or not you have a diagnosis. The three columns organize what you want to explain; they are not levels of severity.
Notice · Adapt · Discuss
Make room for the details that matter to you
Think of one ordinary moment. What happened? Did you change what you did because of sweating? What would you like help with? You can write separate cards for each part, or start with just one.

Bring the wider context too
Be ready to discuss when the sweating started, how often and where it happens, your prescription and non-prescription medicines, and relevant family history. This board focuses on daily-life examples rather than collecting a full medical history.
Sweating with heat or exercise does not by itself establish hyperhidrosis. A clinician can assess your symptoms and discuss appropriate care. You do not need to complete all three columns before asking for help.
Make the most of this tool
Prepare daily-life examples for an excessive-sweating appointment
Write an actual situation, a workaround or something you want help with. Save each example as a card, move it to the column that fits, and put the cards in your preferred order. Print or download the saved notes before your appointment.
Do I need a hyperhidrosis diagnosis to use this worksheet?
No. Use it to explain sweating concerns in your own words. It does not determine whether you have hyperhidrosis or recommend treatment. A clinician can assess your symptoms and the wider context.
Can I move, edit and keep my appointment notes?
Yes. Saved cards can be edited, moved between columns and reordered within a column. Finish or discard a current draft before printing or downloading. The board stays only in the current page’s memory and is not restored after leaving or reloading.
